I'm also having the same problem. I had to contact IA to have them fix one specific item, but there are many others in my large collection and don't know what to do. See page 22 of this item: https://archive.org/details/2016HwySafetyPerfPlan

I wonder if it has something to do with the orientation of the first page (the one perceived by IA's system, which is not always the true first page of the item). I've noticed that if the first page is in landscape orientation, I can view it as well as pages in portrait orientation within the item just fine. But if the (perceived) first page is in portrait, later pages in landscape within the item are cut off. See: https://archive.org/details/2012MOFYEndFundActivityRpt

i'm not seeing pages cut or rotated. i looked at https://archive.org/stream/2016HwySafetyPerfPlan#page/n19/mode/2up

yes, the first page does dictated scaling for the entire book.

As for the first page dictating scaling for the entire document, does that mean that all of the pages should be oriented the same way as the first page in order to prevent cut pages throughout it? I'm thinking of documents where the first page is portrait and later pages are landscape. Do you have any advice to prevent pages from being cut?

it's not cropped. just use the magnification to reduce the size and you'll see the rest of the page.
